We're looking for a Community Link Worker to join our Social Prescribing team and support an innovative new project helping local people overcome barriers to employment.

We're looking for a Community Link Worker to join our Social Prescribing team and support an innovative new project helping local people overcome barriers to employment. The role will focus on working with people who are living with musculoskeletal conditions and/or mental health needs who want support to move towards employment, training or volunteering opportunities.

As a Community Link Worker, you'll build trusted relationships with residents, helping them identify their goals, improve their wellbeing and connect with services and opportunities that can support their journey. You'll work closely with health professionals, employment services, community organisations and voluntary sector partners to provide personalised, practical support.

This is an opportunity to be part of a growing service that has spent almost a decade supporting people across Merton to improve their health, wellbeing and quality of life. You'll join a supportive team, work alongside partners from across the health and voluntary sectors, and help shape an approach that puts people and communities at its heart.

Salary: £31,100 per year
Contract: Full-time, 10-month contract (extension subject to funding), Job share considered.
Location: Merton, with a mix of community-based work, GP practices, outreach locations, our office at Vestry Hall and opportunities for home working.
