Stability Through Starting A New Chapter

Patient A was referred to a social prescriber while facing a combination of complex challenges. These included homelessness, financial hardship, depression and long-term health conditions. Following a family breakdown linked to domestic abuse, she had lost her support network and was living in temporary accommodation, finding it difficult to manage both her wellbeing and day-to-day finances.

The social prescriber worked alongside her over time, taking a person-centred approach to understand what mattered most and where support was needed first. She was connected to domestic abuse services, provided with a food bank voucher, and supported to access financial and housing advice. A joint supporting letter from her GP and social prescriber also helped to strengthen her housing application.

Through consistent, joined-up support, Patient A was able to access a wider network of services. Her social prescriber worked collaboratively with professionals across different organisations to make sure she did not have to navigate this alone.

She shared that she felt happier and more reassured knowing she had someone alongside her, as well as access to organisations she may not have known about or felt confident approaching independently.

With the right support in place, she began taking steps towards a more stable and secure future. This case highlights the value of compassionate, person-centred support in helping people navigate complex situations and access the help they need at the right time.
